Game should be using your highest refresh rate by default unless you have it set not to in your NVidia control panel. In steam, right click left 4 dead 2, properties, set launch options. Type “-refresh 144” or whatever you want it to be. I have my monitor refresh rate forced to 77hz and in nvidia profile inspector set to highest available Network settings, visuals, crosshair, etc I tried with "-autoconfig" but it doesn't work at all, it only resets some settings. I would like to leave everything as default as if I never changed any advanced settings. Figured it out. You have to make sure steam cloud is disabled both in the L4D2 library properties as well as disabling it in the game itself.
